What you will need

• Your PPS number.
• Proof of identity and, usually, proof of address.
• Any supporting documents the form names — medical reports, payslips or bank statements, depending on the payment.
• Details of your income and savings, because this payment is means-tested.

Where to send it

Supplementary Welfare Allowance is handled by the Community Welfare Service at your local Intreo Centre. The official page lists no central address. Find your local Intreo Centre.

After you apply

• Processing takes time. If you are left without income while you wait, you can apply for Supplementary Welfare Allowance in the meantime.
• If you are refused, you can appeal — but you have only 60 days from the date on the decision letter.
• Tell the Department if your circumstances change, as it can affect what you are entitled to.
